I acknowledge this information is correct and I am free from any illness communicable + non – communicable diseases.  I acknowledge that I have been truthfully represented to HOLMES_3D, that I am over 18 (eighteen) years old.  I acknowledge that my pregnancy is less and not more then 6 Months and not under influence of drugs or alchohol.  I acknowledge that it is not reasonably possible for HOLMES_3D to determine whether I might have an allergic reaction to the dyes,pigments or processes used in my tattoo, and medical or skin conditions such as but not limited to: acne, scarring, eczema, psoriasis, moles, I agree to accept that this risk is a possibility.  I acknowledge that infections is always a possibility as a result of obtaining a tattoo,particularly in the event that I do not take proper care of my tattoo.  I acknowledge receipt of the written instructions advising me of the proper care of my tattoo and I recognize the absolute necessity for following those instructions.  I acknowledge that variation in color and design may exist between any tattoo selected by me and as ultimately applied to my body.  I acknowledge that I do not have any physical, mental or medical impairment or disability which might affect my well being as a direct or indirect result of my decision to have a tattoo done at this time I understand that tattoos are permanent and cannot be removed.  I acknowledge that a tattoo is a permanent change to my appearance and that no representations have been made to me as to the ability to later change or remove my tattoo(s).and agree to HOLMES_3D INTERPRETATION.  I acknowledge that the obtaining of my tattoo is by my choice alone and I consent to the application of the tattoo and to any actions or conduct of HOLMES_3D, reasonably necessary to perform the tattoo procedure.  I agree to release and forever discharge and hold harmless to HOLMES_3D, and its representatives, and shareholders from any and all claims,damages or legal actions arising from or connected in any way with my tattoo or procedures and conduct used to apply my tattoo.  I AGREE to give HOLMES_3D consent to publish any video and all photos of my tattoo(s) as for his personal marketing use.  Photo  Video  No Refunds once a tattoo is done.

Cancellation Policy

All appointments require a minimum of 48 hours notice for cancellation. If cancelled with over 48 hours notice, your deposit may be applied to a future appointment. DEPOSITS ARE NON-REFUNDABLE
